Job summary

American Senior Communities is seeking a Memory Care Activity Assistant for Stonebrook Rehabilitation Center. This part-time role focuses on creating meaningful, engaging activities that respect each resident's needs and dignity, delivering warmth and connection daily.

You will work with the care team to plan activities, encourage participation, and support cognitive and physical well-being in a compassionate, person-centered environment.

Qualifications

• Demonstrates C.A.R.E. values: Compassion, Accountability, Relationships and Excellence.
• High school diploma or GED or 1–3 months' related experience and/or training.
• Successful completion of a State approved Nursing Assistant training/certification or CNA program approved by the State.

Responsibilities

• Plan and lead memory care activities for residents.
• Collaborate with care team to support residents holistically.
• Provide compassionate, person-centered engagement to enrich daily living.
