Job summary

Sycamore Heights Health and Rehabilitation is seeking a Memory Care Activities Assistant to support memory unit programs and resident engagement. The role emphasizes planning activities, encouraging participation, and maintaining supplies within a supportive, resident-centered environment.

Qualifications

• Experience in activities preferred.
• Dementia Certification preferred.
• Ability to assume a leadership role in the activities department in the absence of the Memory Director.
• Skill at working with individuals who have cognitive, physical, or sensory disabilities.

Responsibilities

• Assists in planning, coordinating, and evaluating memory unit resident activities.
• Encourages resident participation in programs and activities.
• Escorts residents on activities away from the property as assigned.
• Assists with the registration of residents for trips and programs.
• Maintains necessary activity supplies, which may include outside purchasing.
• Maintains general observation of residents and shares appropriate information with staff.
• Follows departmental budget guidelines.
• Attends all required training, in-service, and staff meetings.
• Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
• Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
• Adheres to all Company policies and procedures
• Performs other duties as assigned.

Essential Functions:

• Assists in planning, coordinating, and evaluating memory unit resident activities.
• Encourages resident participation in programs and activities.
• Escorts residents on activities away from the property as assigned.
• Assists with the registration of residents for trips and programs.
• Conducts various activities at the direction of the Memory Care Director.
• Maintains necessary activity supplies, which may include outside purchasing.
• Assists in the surveying and interviewing of residents to determine interests, talents, and resident satisfaction.
• Maintains general observation of residents and shares appropriate information with staff.
• Follows departmental budget guidelines.
• Attends all required training, in-service, and staff meetings.
• Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
• Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
• Adheres to all Company policies and procedures
• Performs other duties as assigned

Requirements:

• High school diploma or GED required or equivalent related work experience.
• Experience in activities preferred
• Dementia Certification preferred

• Moderate physical activity:
• Requires handling of average-weight objects up to twenty-five (25) pounds.
• Repetitive standing and/or walking for more than four (4) hours per day.
• Repetitive bending and/or stooping for more than one (1) hour at a time.
• Sitting for more than two (2) hours at a time.
• May require consistent computer work with repetitive typing and concentrating on computer screen.
• Physical ability to travel to sites.
