Brio Living Services seeks a Member Navigator (Care Manager) in Grand Rapids, MI, to guide members in accessing healthcare and wellness resources, build trusted relationships, and advocate for aging with dignity at home. This role emphasizes comprehensive assessments, coordination of care, and strong family collaboration.

The position requires healthcare navigation or case management experience, a valid driver’s license, and excellent communication skills.

Qualifications

• Experience in healthcare navigation, case management, or as a medical/health professional is strongly preferred.
• Experience working with the senior population, required.
• Proficiency in medical terminology, required.
• A valid driver’s license, reliable transportation, and proof of auto insurance are required.
• Bachelor’s degree preferred, ideally in a related field.
• Strong interpersonal and communication skills, including active listening and the ability to show empathy.
• Demonstrated ability to develop and implement a holistic, wellness-focused approach to health and well-being for members.

Responsibilities

• Provide personalized, one-on-one guidance to members, assisting them in understanding and accessing available healthcare and wellness services, benefits, and resources.
• Build strong rapport and therapeutic relationships with members, their families, and other healthcare team members.
• Clearly communicate members' rights, options, and responsibilities regarding healthcare and wellness.
• Enhance members’ and families’ health literacy by explaining medications, medical terminology, insurance.
• Conduct comprehensive assessments to identify members’ short- and long-term needs, developing actionable strategies and goals to address those needs.
• Oversee and coordinate caregivers and support staff in delivering high-quality care.
• Regularly monitor members' progress, address concerns, and adjust care plans as necessary.
