Job summary

PacificSource is seeking a dedicated Member Support Specialist to join the case management team. You will work telephonically and in person to help members overcome barriers to adherence and health outcomes, coordinating transportation and linking patients with community resources.

The role involves developing tailored goals with case managers, educating members on benefits, and building strong member-provider relationships to improve care navigation and access.

Qualifications

• Minimum of three years of experience in community services or healthcare coordination.
• Experience in health insurance and delivering group presentations preferred.
• High school diploma or equivalent required.

Responsibilities

• Develop and implement goals and plans with the case manager to help members navigate the healthcare system.
• Educate members on understanding their benefit structure.
• Use motivational interviewing to support member adherence and engagement.
• Identify community resources and make referrals to appropriate services.
• Serve as liaison between members and providers/agencies.
• Coordinate referrals and schedule transportation as needed.
• Ensure HIPAA compliance and accurate documentation.
