Responsibilities

• Follow the nursing process: assess, plan, implement, and evaluate.
• Assessment - gather and analyze data across psychological, physiological, sociocultural, spiritual, economic and lifestyle factors.
• Plan – set measurable short and long term goals with the care team based on the assessment.
• Implement – modify the plan of care, coordinate care, educate and discharge planning.
• Evaluate – regularly review progress and revise the care plan with the team.
• Documents all phases of the nursing process in the patient’s record.
