Qualifications

  • Must pass Criminal Background Check and Health Screening including TB test.
  • Eighteen (18) years old or older required.
  • Current CPR and First Aid certification.
  • Ability to read, analyze, and interpret charting standards and physician orders.
  • Knowledge of Assisted Living and Memory Care regulations.
  • Ability to prioritize, organize and delegate assignments.
  • Completion of state-certified medication aide/tech course.
  • Clear and effective written and verbal communication in English.
  • Commitment to maintaining the integrity of Fields Senior Living.

Responsibilities

  • Provide residents with medication administration and treatment assistance per health care provider orders.
  • Supervise residents who self-administer medication following guidelines.
  • Maintain resident charts and incident reports; observe and report changes to supervisor.
  • Assist residents with ADL tasks including bathing, grooming, toileting, transfers, meals, and housekeeping.
  • Uphold resident rights and interact with residents, families, and staff with courtesy and empathy.
  • Collaborate with other staff to ensure quality of care across departments.
  • Document pertinent information in charts and relevant locations and communicate with physicians as needed.
  • Keep environment safe and clean for residents; observe for health issues and report promptly.
  • Utilize the community communication system and EHR for documentation.
  • Perform other duties as assigned by supervisor.

Essential Functions and Responsibilities:
  • Provide residents with medication administration and treatment assistance and servicesaccordingtothe health care provider’s orders
  • Supervise residents who self-administer medication while following company andregulatoryguidelines
  • Maintain resident records regarding medication distribution and incidents; Observeresidents for safety and reports any and all changes in resident condition effectively,accurately,and in a timely mannertoLN on duty
  • Assisting residents with ADL tasks including but not limited to: assisting with bathing,grooming,toileting,dressing,oralcare,ambulation,transfers,servingandassistingwithmeals,feeding,housekeeping,bedmaking,andlaundry
  • Always observe and uphold resident rights; assist residents to and from meals; Relatetoresidentsin a courteous, empathetic,and cooperative manner
  • Must be willing to work with and provide assistance to other staff from all departments toensure quality of care for residents
  • Documents pertinent information in residents charts and in other specified locations (e.g.; alert charting and incident reports)
  • Communicates with physicians, via fax and telephone
  • Keepenvironment safe andclean for residents
  • Performotherrelateddutiesasassignedbysupervisor
  • Carryandpossesaworkingknowledgeofcommunitycommunication system and EHR.
  • Observes for and reports resident problems and reports such problems to supervisor

Physical Demands and Requirements:
  • Maintain ability to lift 50 pounds; to stand or walk throughout the vast majority of the workday; and to frequently push, pull, bend, twist and reach.
  • Mustbewillingtobeactiveandonfeetduringentireworkshift.
  • Involves light to heavy physical activity performing non-strenuous and strenuous activities.
  • Must have sufficient manual dexterity to work with the fingers and to reach/hand items.
