Trinity Health’s Saint Francis Hospital is seeking a pharmacist to lead quality and compliance, oversee therapy reviews, and guide medication safety in both acute and ambulatory care. You will mentor staff, drive evidence-based practices, and coordinate pharmacy services across units.

The role emphasizes MTM/CMR, education, and program leadership with a focus on safe, affordable care. Licensure in CT is required; residency and certifications are preferred.

Qualificações

• Bachelor’s Degree in Pharmacy; Pharm.D. preferred.
• 3 years’ experience as a pharmacist; residency preferred.
• Board of Pharmacy Specialists certification preferred; CT license.

Responsabilidades

• Lead quality and compliance programs and provide subject matter expertise.
• Design and implement pharmacy-based performance improvement initiatives.
• Oversee medication order review, preparation, distribution, and dispensing.
• Monitor drug therapies to promote optimal use and safety.
• Lead medication safety measures and efficient operations.
• Provide drug information and therapeutic recommendations.
• Perform MTM and comprehensive medication reviews.
• Lead training, mentoring and assessment of competencies.
