Kennybrook Village in Grimes, IA is seeking a Life Services Coordinator - Caregiver to support memory care residents with personal care, meals, activities, and social engagement.

You will work as part of the Maggie's Place Memory Care team, following care plans, supporting independence, and participating in trainings to ensure residents’ well-being and comfort.

Responsibilities

• Provides services to tenants as assigned per Resident Service Plans.
• Reports significant tenant changes to the nurse.
• Monitors tenant needs and health status, documenting changes per procedures.
• Prepares and serves meals and snacks, with proper clean-up.
• Maintains activity calendar and engages tenants in social and recreational activities.
• Upholds safety, infection control, and resident rights.

Shifts

In general we offer 4, 8, and 12 hour shifts for our Caregiver positions. We have openings for full-time (30+ hours weekly) and part-time (up to 29 hours weekly) positions.

• Days: 6am-2:30 pm (Full-Time and Part-Time)

Role

We are seeking a dedicated Life Services Coordinator - Caregiver to join our Maggie's Place Memory Care team! The Life Services Coordinator is responsible for cultivating an environment that maximizes each resident’s independence. The Life Services Coordinator is responsible for delivering assistance with personal care, social and recreational activities, meals, and other tasks as needed to facilitate the continued well-being of tenants.

Responsibilities

• Provides services to tenants as assigned and as indicated on Resident Service Plans which may include grooming and dressing guidance, assistance with bathing.
• Report any significant tenant changes to the nurse.
• Monitors for changes in tenant needs, preferences, and/or health status, and reports/documents any changes according to established procedures
• Reviews the Staff 24-Hour Communication log and corresponding Tenant Service Notes on a daily basis for information about changes in tenant service needs and/or health conditions.
• Prepare and serve meals and snacks, followed by clean-up.
• Monitor food expiration dates.
• Maintain kitchen cleanliness.
• Make beds and change linens.
• Laundry service.
• Housekeeping, to include, dusting and vacuuming, cleaning bathrooms and common spaces.
• Take out garbage.
• Maintain activity calendar and engage tenants in activities such as spiritual, educational, games, music, and physical activities.
• Provide companionship.
• Works effectively with minimal supervision and in a team setting.
• Follows safety policies in performing care, assuring resident safety.
• Follows and upholds policies and procedures.
• Attends and participates in facility meetings and in-service training programs as assigned.
• Follows infection control guidelines.
• Observes residents' rights including, but not limited to, privacy, dignity, etc.
