Legend Senior Living is seeking a dedicated and compassionate Medication Management Nurse to join our team. This crucial role involves overseeing medication administration and ensuring the safe and effective use of medications for residents. Key Responsibilities
- Pass medications on assigned shift.
- Monitor medication administration to ensure accuracy, compliance, and resident safety.
- Conduct routine medication and treatment storage reviews in accordance with policy and procedure.
- Educate residents and caregivers on proper medication use, potential side effects, and adherence to treatment plans.
- Collaborate with healthcare providers, including physicians, pharmacists, and other nurses, to optimize medication therapies.
- Maintain accurate and detailed medication records and documentation.
- Other duties as assigned by Health Care Director

Requirements
- Active Licensed Practical Nurse (LPN) license in Pennsylvania.
- Minimum of 2 years of clinical experience in a healthcare setting, with a focus on medication management preferred.
- Strong understanding of pharmacology, medication administration, and drug interactions.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic medication administration records (eMAR) systems.
Preferred Qualifications
- Experience in personal care, long-term care, hospice, or home health settings.
- Knowledge of regulatory standards and guidelines related to medication management.
- Commitment to continuous learning and professional development.
