Job summary

Corewell Health is hiring a part-time Medication History Technician to obtain accurate medication histories for admitted patients, under pharmacist supervision. You will interview patients or caregivers, document data in the EMR, and flag discrepancies for resolution.

The role requires a high school diploma with pharmacy-related experience or relevant licensure (Pharmacy Technician) and national certification.

Qualifications

Required High School Diploma or equivalent.
Preferred Associate's Degree health care or related field.
1 year of relevant Pharmacy experience required.
2 years of relevant Retail Pharmacy experience preferred.
Ideal candidate will possess full Pharmacy Technician license and national certification (PTCB/ExCPT).

Responsibilities

Provides technical assistance in documenting and clarifying medication histories under the pharmacist's direction.
Processes, enters, and interprets physician/nurse medication histories in the computerized system.
Clarifies information through communication with providers, patients, and pharmacies.
Performs special projects for the department and works with multiple medication automation systems.
Provides professional customer service as a liaison between the pharmacy and patients/caregivers.
