Job summary

LifeBridge Health's Northwest Hospital is seeking a Medication History Technician to obtain and document comprehensive medication histories for admitted patients. You will interview patients, coordinate with external sources, and evaluate adherence to ensure accurate physician reconciliation.

Essential credentials include a valid Pharmacy Technician license and PTCB certification; 3–5 years of related experience is expected.

Job description

Summary

Medication History Technician: Obtains accurate, comprehensive medication history of patients. Interviews patient to obtain complete medication history. Communicates with outside sources to determine all current and active medications prescribed to the patient. Evaluates medication adherence based on patient interview. Accurately documents current medication history for physician reconciliation. This position reports to the Manager of Clinical Pharmacy Services and provides a vital service to our admitted patients and providers.

Credentials

Essential:

• Pharmacy Technician License
• Certified Pharmacy Technician (PTCB)
• Pharmacy Technician (AAH)
• Nationally Registered Certified Pharmacy Technician

Experience

Essential:

• 3-5 years related experience

Pay Range

$22.11 to $33.17 Based On Experience
