Concern Housing in Riverhead, NY is seeking a dedicated Medication Counselor to oversee client medications, assist staff in delivering services, and help run the site. The role includes supervising medication counts, supporting self-management education, and ensuring thorough documentation.

The candidate should have a Bachelor’s degree or 2 years in psychology, rehabilitation, social work, or another mental health discipline.

Qualifications

• Bachelor’s degree or 2 years’ experience in psychology, rehabilitation, social work or other mental health discipline.
• Duties include medication supervision, counts, and documentation; report discrepancies.
• Provide wellness self-management education to residents for independence in self-medication.
• Provide direct care services to clients.
• Document client progress and maintain unit records.
• Monitor site and coordinate house activities (meals, chores, safety checks).
• Deliver rehabilitative support in daily living skills and health services.
• Attend supervision and staff meetings weekly.
• Remain current with agency compliance documents and trainings.

Responsibilities

• Performs unit inspections, assists residents with upkeep and cleanliness of individual units.
• Attends weekly supervision and staff meetings.
• Other duties, as assigned by Program Director/ Supervisor.
• Remain current with all Agency required compliance documents and trainings.

Description

Concern Housing is seeking aMedicationCounselorto beresponsible for providing oversight and monitoring of clientmedications and assisting staff in the delivery of direct services to the clients, as well as assisting in the overall running of the site. Thecounselorprovides a variety of services, ranging from the teaching of skills to delivery of concrete services to providing counseling to the clients.

Days/Hours:Monday - Friday; 4pm - 12am

Duties and Responsibilities:

• Supervisesmedicationand completesmedicationcounts with evening and overnight staff as needed (daily for controlled substance, weekly for all othermedications). Reports discrepancies immediately to supervisor; completes all corresponding documentation
• Provides wellness self-management education to residents to assist with independence in self-medicationmanagement.
• Provides direct care services to clients.
• Responsible for documentation of clients assigned to as well as documentation relating to other clients’ progress.
• Monitors the site, coordinates and participates in house activities such as preparation of meals, chore activities, recreational activities, safety checks etc.
• Provide rehabilitative support services in daily living skills, health services and others as assigned.
• Performs unit inspections, assists residents with upkeep and cleanliness of individual units
• Attends weekly supervision and staff meetings.
• Other duties, as assigned by Program Director/ Supervisor.
• Remain current with all Agency required compliance documents and trainings.

Requirements: Bachelor’s degree or 2 years’ experience in a Psychology, Rehabilitation, Social Work or other mental health discipline.
