Job summary

Community Health Network is seeking a Medication Assistance Program Coordinator to support ambulatory care pharmacy activities, manage the Medication Assistance program, and coordinate patient follow-up in the Kokomo region. Training begins at the Greenwood hub with 4–6 weeks on-site, followed by a phased hybrid schedule and a minimum of 2 days on-site at the Kokomo office as needed.

The role requires a high school diploma or GED and licensure or certification options such as Indiana Pharmacy

Qualifications

• High school diploma or GED required.
• Indiana State License as Pharmacy Technician and Certified Pharmacy Technician through the PTCB or NHA, or Medical Assistant Certification through CMA(AMMA), NCMA (NCCT), RMA (AMT), CCMA (NHA) or NRCMA (NAHP).
• Multiple eligible certifications may be accepted as listed above.

Responsibilities

• The Medication Assistance Program Coordinator supports the clinical activities of ambulatory care pharmacy and oversees program management and patient follow-up coordination.

Education

High school diploma or GED

Indiana Pharmacy Technician License or PTCB/NHA certification

Medical Assistant Certification (CMA/AMMA, NCMA/NCCT, RMA/AMT, CCMA/NHA, NRCMA/NAHP)
