Agape Behavioral Health seeks a Telehealth Therapist for our South Florida facilities. The role involves group, individual, and family therapy, psychosocial assessments, and collaboration with the treatment team to plan discharge and treatment strategies.

The qualified candidate holds a valid Florida license (LMHC, LCSW, LMFT) and has at least 1 year of experience in substance abuse or mental health treatment. Remote work arrangements apply with supervision by the Clinical Director.

Qualifications

• Master's degree required.
• LCSW/LMHC/LMFT license required.
• Minimum 1 year experience in substance abuse or mental health treatment preferred.

Responsabilités

• Review assessments from referring facilities and note problems requiring attention.
• Contact referral sources to gather information and maintain contact through treatment.
• Gather diagnostic information via psychosocial assessment and develop an Integrated Summary.
• Develop a comprehensive problem list reflecting patient issues and present to the treatment team.
• Review initial treatment plan during first session and ensure assignments are clear.
• Formulate a comprehensive, measurable treatment plan with patient and establish goals.
• Participate in multidisciplinary treatment plan reviews with preparation and clinical judgment.
• Develop interventions with planned frequency and monitor progress.
• Maintain clinical records per policy; ensure timely, clear, secure documentation.
• Continue planning throughout stay, updating plans as needed.
• Lead and co-facilitate group therapy sessions to foster self-disclosure and group skills.
• Complete activity and group plans for the Clinical Director.

herapist for our facilities in South Florida!

The

Telehealth

Therapist is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. He/she will be responsible for patient caseload and will provide group, individual, and family therapy. The Telehealth Therapist will conduct psychosocial and bio-psychosocial assessments. He/She will collaborate with the Treatment Team to formulate the Master Treatment Plan and assist in coordinatingdischarge planning. The Primary Therapist has primary responsibility for maintaining communication with the family, referral source and other service providers as neededLocation: Remote, Must have a valid clinical license for the state of Florida (LMHC, LCSW, LMFT).

Specific Areas of Responsibility:

• Review assessments from referring facilities and note problems/issues that require further attention
• Contact referral source and other key individuals to gather additional information as necessary and maintain contactwith referral source and other key individuals throughout patient’s treatment
• Gather comprehensive diagnostic information through psychosocial assessment interview then develop anIntegrated Summary, bringing together information from all assessments to identify problem areas that may affecttreatment and/or need to be addressed in treatment planning
• Develop a comprehensive problem list that reflects patient’s individual issues as described in assessments and theIntegrated Summary and present the psychosocial and diagnostic information to treatment team in a clear, concisemanner
• During first session with patient, review initial treatment plan, including case management issues and be sure thatpatient and staff assignments and responsibilities are understood and in progress
• Formulate a comprehensive treatment plan with patient that effectively addresses patient issues has clearly statedtreatment goals and objectives that are measurable and can be completed within established time frames
• Participate effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation andunderstanding of patient issues, progress, strengths and weaknesses, areas of resistance, positive and negativestrategies, and use good clinical judgment in moving patient through treatment levels
• Develop interventions that will accomplish goals and objectives and state the planned frequency of interventions
• Maintain clinical records according to program policies and those of licensing and accrediting agencies, documentthe patient’s progress accurately as it relates to treatment goals and objectives, ensure that documentation is timely,consistent, clear, and articulate and keep clinical records always secure.
• Continue treatment planning process throughout patient’s stay in treatment, opening new problems and developingor revising treatment plans as necessary
• Lead and co-facilitate group therapy sessions in a manner that creates an atmosphere in which self- disclosure isfacilitated, helps patients gain group process skills, and helps patients gain insight into problems and utilizesexperiential techniques effectively
• Complete activity and group plans for the Clinical Director including supplies utilized and/or needed

Requirements:

• Masters Degree Required
• LCSW, LMFT, LMHC Required
• A minimum of 1 year experience in substance abuse or mental health treatmentpreferred.
