Job summary

Responsibilities

• Interviews patient and family to assess home and social situation.
• Evaluate data to align with medical plan and social services.
• Educate families to understand and follow medical recommendations.
• Coordinate transportation, equipment, and reduce patient/family anxieties.
• Assist patient to resume community living and manage chronic illness.
• Prepare and maintain patient records and case documentation.
• Represent social work on multidisciplinary care teams.
• Collaborate with clinicians to ensure coordinated care and discuss advance directives.
