Formación

• Master's in Social Work from an accredited program.
• Active Social Worker license in the state of practice upon hire.
• QMHP (Qualified Mental Health Provider) preferred upon hire.
• 1–3 years of related experience.

Responsabilidades

• Interviews patients and families to assess home environment, health history, and psychosocial needs.
• Evaluates data against medical plan to plan therapy and referrals.
• Aids patients and families to understand and follow medical recommendations for compliance.
• Coordinates discharge planning and placement into post-acute care when needed.
• Maintains EMR documentation and supports quality assurance within the department.
• Contributes to care conferences and multi-disciplinary team discussions.
