Job summary

ABL Home Health in Concord, CA is seeking a Medical Social Worker (MSW) to support patients and families at home, coordinating with clinical teams to address social determinants of health.

The MSW will assess needs, provide counseling, connect to resources, document care plans, and ensure regulatory compliance. This full-time role offers meaningful work with a supportive team.

Qualifications

• Master's degree in Social Work (MSW) required.
• 2+ years of MSW experience in a Home Health or similar setting required.
• Strong interpersonal, organizational and counseling skills.
• Valid Driver's License and reliable transportation.

Responsibilities

• Assess the psychosocial needs of patients and families to develop individualized care plans.
• Provide counseling and emotional support to patients and caregivers.
• Assist in connecting patients and families to community resources, financial assistance, and legal aid.
• Collaborate with nurses, therapists, and physicians to ensure comprehensive care planning.
• Document all services provided in compliance with agency policies and regulatory requirements.
