Job summary

ABL Home Health in Concord, CA is seeking a Medical Social Worker (MSW) to join our home health team on a part-time basis. The MSW will assess psychosocial needs, provide counseling to patients and families, connect with community resources, and collaborate with nurses, therapists, and physicians to support comprehensive care.
