Perform initial evaluations during the admission process to assess the patient’s psychosocial status.
Identify socioeconomic and emotional factors that may impact the patient’s treatment plan.
Collaborate with the interdisciplinary team to develop and implement comprehensive patient care plans.
Consult directly with attending physicians regarding any necessary changes to treatment.
Maintain and submit accurate, timely clinical documentation (evaluations, care plans, progress notes).
Provide monthly supervision to the social work assistant.
Current Licensed Clinical Social Worker (LCSW) credential in California.
Valid California driver’s license and reliable transportation.
Current First Aid and CPR certification.
Coverage Area: South Bay and surrounding communities.
