Job summary

Health Link Home Health and Hospice in the South Bay area is seeking a compassionate Medical Social Worker (MSW) to join our team on a per-diem basis. You will evaluate psychosocial needs, coordinate with the interdisciplinary team, and support patients and families through care planning.

This role emphasizes collaboration, timely documentation, and adherence to California LCSW standards. A valid CA driver’s license, First Aid/CPR, and flexible scheduling help you deliver high-quality home

Qualifications

• Current LCSW credential in California.
• Valid California driver’s license and reliable transportation.
• Current First Aid and CPR certification.

Responsibilities

• Perform initial psychosocial evaluations during admission.
• Identify socioeconomic and emotional factors affecting treatment.
• Collaborate with the interdisciplinary team to develop care plans.
• Consult with attending physicians regarding treatment changes.
• Maintain and submit accurate clinical documentation (evaluations, care plans, progress notes).
• Provide monthly supervision to the social work assistant.

Key Responsibilities

• Perform initial evaluations during the admission process to assess the patient’s psychosocial status.

Requirements
