Job summary

Health Link Home Health and Hospice Agency is seeking a compassionate Medical Social Worker (MSW) on a per-diem basis for the South Bay area. The role emphasizes psychosocial assessments, care planning, and collaboration with a dedicated interdisciplinary team in a supportive field-based setting.

Qualified applicants will hold a California LCSW, a valid CA driver’s license, and current First Aid/CPR certification. Mileage reimbursement and flexible scheduling are among the benefits offered.

• Current Licensed Clinical Social Worker (LCSW) in California.
• Valid California driver’s license and reliable transportation.
• Current First Aid and CPR certification.

Responsibilities

• Perform initial evaluations during the admission process to assess the patient's psychosocial status.
• Identify socioeconomic and emotional factors that may impact the patient's treatment plan.
• Collaborate with the interdisciplinary team to develop and implement comprehensive patient care plans.
• Consult directly with attending physicians regarding any necessary changes to treatment.
• Maintain and submit accurate, timely clinical documentation (evaluations, care plans, progress notes).
• Provide monthly supervision to the social work assistant.
