Job description

We Are Hiring
  • Department: Case Management
  • Schedule: As Needed; weekdays, weekends, and holiday rotation
  • Hospital: Saint Joseph Hospital
  • Location: Chicago, IL
  • Salary: $40.00 per hour
What You Will Do

Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge.

  • Create care plans across the continuum, incorporating patient and family preferences and values.
  • Monitor patient care through assessments, evaluations, and review of patient records.
  • Advocate for resources and work to remove barriers to care.
  • Maintain ongoing communication with supervisors and other healthcare providers to ensure effective implementation of the health plan.
  • Serve as a resource for proper medical record documentation, appropriateness of services related to diagnoses, and post-discharge treatment options.
What You Will Need

Licensure / Certification / Registration:

  • One or more of the following required:
    • Clinical Counselor credential from the Illinois Department of Financial and Professional Regulation obtained prior to hire or transfer.
    • Counselor credential from the Illinois Department of Financial and Professional Regulation obtained prior to hire or transfer.
    • Social Worker specializing in Clinical Practice credential from the Illinois Department of Financial and Professional Regulation obtained prior to hire or transfer.
    • Social Worker credential from the Illinois Department of Financial and Professional Regulation obtained prior to hire or transfer.

Education:

  • High school diploma or equivalent with 2 years of experience, or an Associate's/Technical degree, or 4 years of relevant experience.
Additional Preferences

*Must have LSW/LCSW or LCPC certification.

*Availability to work one day shift per week and two holidays per year.

**Reside within 30 minutes of the hospital.
