Job summary

Trinity Health At Home is seeking a compassionate Medical Social Worker (PRN) for part-time night shifts to support homebound patients and their families. You will conduct psychosocial assessments, develop person-centered care plans with the interprofessional team, and help remove barriers to care while promoting independence and improved quality of life.

The role requires an MSW from a CSWE-accredited program, state licensure, CPR certification, and at least two years in healthcare social work.

Qualifications

• Master's Degree in Social Work from CSWE-accredited program.
• Current Master of Social Work license as required by state regulations.
• Current CPR certification.
• Minimum of two (2) years of social work experience in a healthcare setting.
• Ability to provide services to acutely and chronically ill patients.
• Strong communication, assessment, and critical thinking skills.
• Commitment to patient confidentiality and professional ethics.

Responsibilities

Position Purpose

Trinity Health at Home is seeking a compassionate and skilled Medical Social Worker (PRN) to provide consultation, assessment, and support services to homebound patients and their families. In this role, you will help address complex social and emotional challenges that may impact a patient’s ability to manage their medical condition, promoting improved health outcomes, independence, and quality of life.

What You Will Do

• Conduct comprehensive psychosocial assessments of patients and caregivers.
• Develop and implement individualized, person-centered plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.

Minimum Qualifications
