Qualifications

• Master’s degree of Social Work from an accredited School of Social Work.
• 1 year social work experience in a health care setting.
• Temporary social work licensure in the state of Wisconsin.
• Flexible in responding to changing needs; demonstrates communication and problem-solving skills.
• Competency in accessing and documenting in patient documentation systems; basic computer skills.

Responsibilities

• Assist with identification of patients with complex or special discharge needs and collaborates with the interdisciplinary team in care plans.
• Provide psychosocial services to diverse patient populations to promote appropriate utilization of health care resources and outcomes.
• Support transition planning and facilitate timely licensure progress toward permanent Medical Social Worker roles.
