Job summary

Ohio Hospice is seeking a Medical Social Worker to provide direct patient care in Lancaster, Ohio and surrounding communities. The role is primarily field-based, with patient home and facility visits in a full-time capacity.

Responsibilities include evaluating psychosocial needs, developing interventions, and coordinating with interdisciplinary hospice teams. Travel is required across the assigned service area, including on-call shifts during weekends and holidays.

Qualifications

• Bachelor's degree in Social Work required.
• Active Ohio LSW or LISW license.
• At least one year of social work experience in a healthcare environment.
• Valid driver's license and automobile insurance meeting state requirements.
• Ability to travel between patient homes and facilities.
• Comfortable using electronic systems for clinical documentation.

Responsibilities

• Offering emotional support, counseling, and crisis assistance.
• Helping families navigate community resources and available services.

• Participating in the organization's on-call, weekend, and holiday rotation.

We are looking for a Medical Social Worker to serve patients throughout Lancaster, Ohio and the surrounding area. This position provides an opportunity to build meaningful relationships with patients and families while working as part of a collaborative hospice care team.

Your Role

As a member of the hospice team, you'll help patients and families identify their needs and connect them with appropriate support and resources.

Qualifications

Required

Bachelor's degree in Social Work

Active Ohio LSW or LISW license

At least one year of social work experience in a healthcare environment

Valid driver's license

Ability to travel between patient homes and facilities

Comfortable using electronic systems for clinical documentation

Additional Experience We Value

MSW or other Master's degree in Social Work

Previous hospice or palliative care experience

CHP-SW or ACHP-SW certification

Current CPR certification

Notary designation or willingness to obtain one within six months of starting
