MSA Home Health & Hospice in Virginia seeks a Full-Time Medical Social Worker to provide compassionate end-of-life care to patients and families in the Bluefield, Richlands, and Grundy areas.

You will collaborate with the interdisciplinary team to assess needs, coordinate services, and support bereavement when appropriate. A CSWE-accredited degree and VA licensure are required; hospice experience preferred and a valid driver's license is essential.

Qualificações

• MSW or BSW from CSWE-accredited program.
• Currently licensed to practice social work in VA.
• Minimum of one year hospice experience preferred.
• Valid driver's license.

We're currently seeking a dedicated and experienced Full-Time Medical Social Worker to join our team and provide meaningful, end-of-life care to patients and families in the Bluefield, Richlands, & Grundy (Tazewell, Buchanan), VA areas.

What You’ll Do:

• Provides social work services in accordance with the physician’s plan of treatment.
• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
• Serves as a liaison with other community resources.

Job Requirements

• Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
• Currently licensed to practice as a social worker in the state of VA.
• Minimum of one-year Hospice experience preferred.
• Valid Driver's License.
