Job summary

Oak Street Health is seeking a Medical Social Worker to support psychosocial needs within Centers. You will perform assessments, advocate for patients, and coordinate care with interdisciplinary teams to help high-risk patients and their caregivers improve health outcomes and avoid hospitalizations.

You will collaborate with patients, families, and care teams, navigate insurance benefits, and provide education on advance care planning and goals of care at all stages of the journey.

Qualifications

• MSW degree from a CSWE-accredited program.
• Licensed or minimally licensed in the state where OSH Center is located.
• US work authorization and ability to work in the United States.

Responsibilities

• Provide assessments, advocacy, and care coordination for high-risk patients.
• Collaborate with care teams to identify goals and support care plans.
• Refer to community resources and navigate insurance benefits.
• Educate patients and families on care options and goals of care.
• Document interventions to ensure team visibility and continuity of care.
