Job summary

Oak Street Health seeks a Medical Social Worker to support psychosocial needs of patients within Oak Street Health Centers. You will perform assessments, advocacy, and comprehensive care coordination for high-risk patients and caregivers to improve health and reduce hospitalizations.

You will collaborate with an interdisciplinary team, educate patients on care options, navigate insurance benefits, and document interventions to keep the care team informed.

Qualifications

• Must hold a Master of Social Work degree from a CSWE-accredited program.
• Licensed in the state where the OSH Center is located.
• Fluent in Spanish, Polish, Russian, or other languages relevant to the communities served.
• Proficient in Microsoft, G Suite, and electronic medical records (EMR).
• US work authorization is required.

Responsibilities

• Provide comprehensive assessments, advocacy, and care coordination for high-risk patients and caregivers.
• Collaborate with interdisciplinary care teams to support psychosocial needs and improve health outcomes.
• Navigate insurance benefits and resources to maximize plan benefits and access to care.
• Document interventions timely so the care team has visibility into MSW activities.
• Educate patients on supportive care options, goals of care, and end-of-life planning.
