Job summary

Palms-Home-Care is seeking a compassionate Medical Social Worker to support home health patients and their families by addressing psychosocial, emotional, and resource needs. This role emphasizes empathy, clear communication, and effective care coordination in a home health setting.

The successful candidate will hold an MSW, have an active Florida license (LMSW/LCSW preferred), and demonstrate strong knowledge of community resources, with experience in home health or healthcare.

Qualifications

• MSW required with Florida licensure or eligibility.
• Active Florida license (LMSW/LCSW preferred).
• Home health or healthcare experience preferred.

Responsibilities

• Assess patient psychosocial and environmental needs
• Assist with discharge planning and long-term care arrangements
• Provide counseling and emotional support to patients and families
• Connect patients with community resources and financial assistance programs
• Complete required documentation per Medicare/home health standards
• Participate in care coordination with interdisciplinary team
