Life Care Home Health is seeking a Medical Social Worker to provide coordinated in-home care across all age groups, performing psychosocial assessments, counseling and referrals to meet client and family needs.

You will collaborate with an interdisciplinary team, participate in case conferences, maintain licensure and CPR certifications, and support compassionate, patient‑ and family‑centered care in a home-health setting.

• Strong decision-making and documentation skills.

Responsabilités
