Job summary

Apploi is seeking a compassionate Medical Social Worker to support home health patients and their families in Orlando, Florida. You will assess psychosocial needs, assist with discharge planning, and connect patients with community resources and financial assistance.

This role requires strong communication and organizational skills, a Florida LMSW/LCSW license preferred, and prior home health or healthcare experience. A Level 2 background screening is required before employment.
