Job summary

Qualifications

• Masters of Social Work required.
• Previous experience providing social work/case management services preferred.
• Experience in a hospital environment preferred.

Responsibilities

• Discharge planning and coordinating care with the Clinical Care Coordinator and care teams.
• Assess psychosocial needs of patients and families and address social determinants of health.
• Coordinate homecare and ensure a smooth transition from hospital to next level of care.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
