Job summary

Oak Street Health seeks a Medical Social Worker to support psychosocial needs of our high‑risk patients. You will perform assessments, advocate for patients, and coordinate care with the interdisciplinary team to improve health outcomes.

Responsibilities include home visits, care coordination, crisis response, and patient education on advance care planning. Requires MSW (CSWE) and state licensure, with strong communication skills.

Qualifications

• MSW degree from CSWE-accredited program.
• Licensed in the state where OSH Center is located (LCSW/LSW).
• Fluency in Spanish/Polish/Russian or other community languages if needed.
• Proficient with MS Office, GSuite and electronic medical records.
• US work authorization.
• Experience with patient/home visits.

Responsibilities

• Provide comprehensive assessments and telephonic/in-person check-ins for high-need patients.
• Coordinate with interdisciplinary team to develop and execute care plans.
• Navigate insurance benefits to maximize coverage and minimize gaps.
• Educate patients and families on advance care planning and care options.
• Document interventions timely for care team visibility.
• Respond to crises and manage multiple priorities.
