Oak Street Health is seeking a Medical Social Worker to support psychosocial needs of patients across centers. You will assess, advocate, and coordinate care with a multidisciplinary team to improve health outcomes and reduce avoidable hospitalizations.

Responsibilities include comprehensive assessments, insurance navigation, care planning, and patient education. Strong communication, licensure, and language skills are essential for success in a diverse patient population.

Formación

• MSW degree from a CSWE-accredited school.
• Active professional licensure in the state where the OSH Center is located.
• Fluency in multiple languages as needed by patient population.
• Proficiency with standard office software and EMR systems.
• U.S. work authorization.

Responsabilidades

• Provide assessments, advocacy, and care coordination for high-risk patients and caregivers.
• Collaborate with interdisciplinary care teams to support psychosocial needs.
• Conduct telephonic and in-person visits and maintain documentation.
• Navigate insurance benefits and maximize plan utilization.
• Offer care planning, goals of care counseling, and crisis response.
• Educate patients and families on end-of-life care and support options.
• Coordinate with Community Health Workers and provide clinical guidance.
• Maintain timely, concise documentation for care team visibility.
