Job summary

Hospice of South Texas is seeking a compassionate Medical Social Worker to support patients and families through end-of-life care. Eligible candidates include LBSW, LMSW or LCSW with at least one year of healthcare experience.

You will conduct psychosocial assessments, provide counseling and resource coordination, and participate in interdisciplinary team meetings. Employer-provided CEUs are available and onboarding is supportive.

Qualifications

• LBSW/LMSW or LCSW license in Texas is required.
• Minimum of one year of experience in a healthcare setting.
• Strong communication and interpersonal skills.
• Ability to manage emotionally complex situations with professionalism.

Responsibilities

• Complete psychosocial assessments for hospice patients and families.
• Provide emotional support, counseling, and resource coordination.
• Participate in interdisciplinary team meetings.
• Assist with care planning, coping strategies, and end-of-life support.
• Maintain timely and accurate clinical documentation.
