Job summary

JourneyCare Hospice in Vernon Hills, IL is seeking a Hospice Social Worker to join our growing team. The role focuses on psychosocial assessment, care planning, and helping patients and families navigate hospice services.

Qualifications

• MSW required; social work degree essential.
• Current State Licensure in Social Work is required.
• Minimum of one year’s experience in health care or hospice setting.

Responsibilities

• Perform psychosocial assessments of patients and families in timely fashion and integrate needs into the plan of care.
• Provide exceptional service to patients, families, referral sources, and co-workers; support team as needed.
• Counsel patients and families on stress and coping, when appropriate.
• Document patient services accurately and promptly to reflect the plan of care.
• Assist with applications for SSI, Hospice, Medicaid, and Medicaid Waivers; connect with community resources.
• Serve as liaison between patients, families, and community agencies.

On-Call Expectations

3 shifts/month On-Call Rotation Afterhours and Weekends: Weeknights: 5PM-8AM and Weekends: 8AM-5PM and 5PM-8AM

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Salary Range: $62,000-$80,000 - Offer based on experience
