Job summary

VNHC in Santa Barbara, CA, is seeking a Medical Social Worker to deliver psychosocial assessments and support within home health services. This full-time role involves coordinating with medical staff, families, and community agencies to ensure comprehensive care across the patient’s plan.

Ideal candidates hold a Master's in Social Work, at least one year in health care, and a valid driver’s license. Strong verbal and written communication, plus organizational skills, are essential for success

Qualifications

  • Master's Degree in social work from a CSWE-accredited program.
  • Minimum of one year of experience in health care.
  • Experience in a home health care setting preferred.
  • Licensed driver with insured automobile.
  • Demonstrates good verbal and written communication, and organization skills.

Responsibilities

  • Assess psychosocial status of patients related to illness and environment.
  • Conduct social evaluations and plan interventions based on evaluation findings.
  • Maintain clinical records on all patients referred to social work.
  • Provide information and referral services for patients and families/caregivers.
  • Provide information to patients or families/caregivers and community agencies.
  • Liaise between patients or families/caregivers and community agencies.
  • Maintain collaborative relationships with organization personnel to support patient care.
  • Develop and maintain contracts with public and private agencies as resources for patients and staff.
  • Participate in development of total care plan and attend case conferences as required.
  • Assist physicians and other team members in understanding social and emotional factors related to health problems.
  • Meet average daily productivity standards and stay informed about applicable regulations.

Job Summary

The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. Respects and promotes the mission, vision, and values of VNHC.

Job Duties:

  • Assesses the psychosocial status of patients related to the patient’s illness and environment.
  • Carries out social evaluations and plans intervention based on evaluation findings.
  • Maintains clinical records on all patients referred to social work.
  • Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
  • Provides information to patients or families/caregivers and community agencies.
  • Serves as liaison between patients or families/caregivers and community agencies.
  • Maintains collaborative relationships with Organization personnel to support patient care.
  • Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
  • Participates in the development of the total plan of care and case conferences as required.
  • Assists physician and other team members in understanding significant social and emotional factors related to health problems.
  • Meets the average daily productivity standards as set by the agency.
  • Knowledge of state, federal, local and accreditation regulations pertaining to the delivery of home health services.
  • Other duties as delegated by the Director of Home Health/Clinical Supervisor.

  • Ability to push/pull 50 lbs.
  • Ability to stoop, bend, and squat.
  • Ability to walk up one flight of stairs.
