LiveWell Homecare Agency, LLC is seeking a Medical Social Worker to apply social work expertise and clinical knowledge to educate patients living with kidney disease about treatment, health, and wellness to improve their quality of life.

You will conduct psychosocial assessments, participate in care planning, and manage services to help patients cope with chronic kidney disease and dialysis treatments, collaborating with a multidisciplinary team.

Qualifications

• Master’s Degree in Social Work from an accredited institution.
• MSW required; LCSW/ACSW preferred (LCSW in CA, licensure in TX/TN).
• Preferable experience in healthcare or behavioral health settings.
• Strong interpersonal and compassionate communication skills.
