Job summary

Cabrillo Hospice is seeking a compassionate Social Worker to join our interdisciplinary team in California. You will conduct psychosocial assessments, provide emotional support, and collaborate with nurses, physicians, and chaplains to tailor care plans for patients and families.

You will connect families to community resources, participate in bereavement services, and ensure compliance with regulatory standards, while delivering patient-centered end-of-life care.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Hospice or healthcare experience preferred (geriatrics or end-of-life care a plus).
• Strong interpersonal and communication skills.
• Compassionate, patient-centered approach.
• Ability to work independently while collaborating within a multidisciplinary team.
• Cultural sensitivity and respect for diverse backgrounds and belief systems.

Responsibilities

• Conduct Comprehensive Psychosocial Assessments: Assess the emotional, social, and environmental needs of patients and families, including caregiver stress, family dynamics, coping abilities, and safety concerns.
• Provide Emotional Support & Counseling: Offer compassionate counseling to patients and families navigating serious illness, grief, and end-of-life transitions.
• Collaborate with the Interdisciplinary Team: Work closely with nurses, physicians, chaplains, and other care team members to develop and support individualized plans of care.
• Connect Families to Community Resources: Serve as a liaison to community agencies and support services to ensure patients and families have access to appropriate resources.
• Bereavement Support: Participate in and support hospice bereavement services, providing follow-up and resources to families after the loss of a loved one.
• Ensure Compliance & Quality Care: Maintain accurate documentation and ensure services align with hospice regulations, policies, and standards of care.
