Responsibilities

• Conduct Comprehensive Psychosocial Assessments for patients and families, addressing emotional, social, and environmental needs.
• Provide Emotional Support & Counseling to patients and families navigating serious illness and grief.
• Collaborate with the Interdisciplinary Team to develop and support individualized plans of care.
• Connect Families to Community Resources and support services.
• Participate in and support hospice bereavement services for families.
• Maintain accurate documentation and ensure services align with hospice regulations and standards.

What You’ll Do

• Conduct Comprehensive Psychosocial Assessments Assess the emotional, social, and environmental needs of patients and families, including caregiver stress, family dynamics, coping abilities, and safety concerns.
• Provide Emotional Support & Counseling Offer compassionate counseling to patients and families navigating serious illness, grief, and end-of-life transitions.
• Collaborate with the Interdisciplinary Team Work closely with nurses, physicians, chaplains, and other care team members to develop and support individualized plans of care.
• Connect Families to Community Resources Serve as a liaison to community agencies and support services to ensure patients and families have access to appropriate resources.
• Bereavement Support Participate in and support hospice bereavement services, providing follow-up and resources to families after the loss of a loved one.
• Ensure Compliance & Quality Care Maintain accurate documentation and ensure services align with hospice regulations, policies, and standards of care.

What We’re Looking For

• Master’s Degree in Social Work (MSW)
• Hospice or healthcare experience preferred (geriatrics or end-of-life care a plus)
