Job summary

Qualifications

• Master's or Doctoral Degree in Social Work from a CSWE-accredited program.
• Two (2) years of Social Work experience in a Home Health setting.
• Active CPR certification.
• 1-2 years of experience supervising clinicians in a Home Health Agency environment.
• Experience using Electronic Medical Records (EMR).

Responsibilities

• Provide social work services to patients on an intermittent basis in their place of residence.
• Assess patient/family system needs.
• Develop and implement treatment plans in accordance with departmental and medical center policies.
• Perform bio-psychological assessment interviews with patients, families, or significant others.
• Provide crisis intervention and treatment to adults, children, and families in emergency situations.
• Provide assessment, counseling, information/referral, and consultation to physicians and the healthcare team.
• Participate in development of the Plan of Care and prepare clinical and progress notes.
• Submit assessments and notes timely.
• Interpret patients' conditions professionally and recommend appropriate psychosocial intervention and/or treatment plans.
• Provide services under physician orders and the plan of care, under supervision of the Director of Clinical Services/Nursing Supervisor.
