Job summary

Jobtailor is seeking a Medical Social Worker to provide psychosocial support to patients and families in a healthcare setting. You will perform psychosocial assessments, empower clients, and collaborate with clinical staff to address family dynamics impacting health problems.

Documentation must be timely and accurate. Responsibilities include developing individualized Plans of Care, attending Case Conferences, and engaging with both adult and pediatric patients.

Qualifications

• Master’s degree in Social Work (CSWE accredited) required.
• Minimum 1 year of health care social work or counseling internship.
• Experience with adults and pediatric patients concepts.
• Experience using an EMR/EHR system.
• Valid driver's license and automobile insurance for client home visits.

Responsibilities

• Provide psychosocial support to patients and families.
• Perform psychosocial assessments and develop Plans of Care.
