Job summary

Responsibilities

  • Complies with ethical and professional standards of conduct as set forth by the National Association of Social Workers.
  • Maintains patient safety and dignity by adhering to HIPAA and related policies.
  • Provides rehabilitative and supportive casework to restore patients to optimum social and health adjustment.
  • Assists patients and families with personal and environmental difficulties affecting care.
  • Prepares social histories and Plans of Care to improve services.
  • Assists patients and families in utilizing home resources or referrals to community agencies.
  • Assesses family finances considering prognosis and medical needs, facilitating financial assistance referrals.
  • Collaborates with physicians and health team to address social, emotional and environmental factors.
  • Documents progress notes in the patient’s chart weekly.

Position & Responsibilities:
  • Complies with ethical and professional standards of conduct as set forth by the National Association of Social Workers.
  • Practices and maintains patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).
  • Provides rehabilitative and supportive casework with the purpose of restoring patients to their optimum level of social and health adjustment. This includes assisting patients and their families to understand, accept and adhere to medical regimens.
  • Assist patients and their families with personal and environmental difficulties which predispose them to illness and interfere with their obtaining maximum benefits from medical care.
  • Prepares a social history and Plan of Care with the purpose of improving the scope and quality of existing services or extending services. When indicated, assists in the development of the Physician's Plan of Care.
  • Assist patient and family to utilize existing resources in the home environment or refer patient and family to community agencies and act as an intermediary for the patient as necessary.
  • Assess, when appropriate, a family's financial situation, taking into consideration the patient’s prognosis and medical needs and facilitating the referral process to any agency for financial assistance.
  • Consults with the Physician and other members of the health team for the purpose of assisting them to understand significant social, emotional and environmental factors related to the patient’s health problems.
  • Records clinical/progress notes in the patient’s clinical record weekly.

Requirements
Qualifications:
  • Degree in Social Work (Minimum of Bachelor's degree in Social Work or Psychology)
  • LSW license in good standing in the State of Colorado
  • Documented evidence of training in dealing with grief and loss.
  • Hospice Experience (Required)
  • Bachelor's in Psychology or Social Work
  • Social Work License (Preferred)
