Job summary

CommonSpirit Health at Home is seeking a Medical Social Worker to join our team delivering compassionate care in patients' homes. You will establish care plans, provide crisis intervention, and support families while collaborating with physicians and caregivers.

Qualifications

• Master’s degree in Social Work required.
• State licensure to practice in Indiana (IC25-23-6-4-3) required.
• One year of social work experience in a health care setting.
• Current CPR/BLS certification from AHA or ARC and state compliance.

Responsibilities

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting physicians and other caregivers in understanding social, emotional and economic factors influencing patient improvement.
• Providing patient care education to patients and caregivers.
• Assisting with community referrals for public assistance, housing, resources, transportation.
• Collaborating with patient’s caregivers to resolve problems affecting care relationships.
• Completing and submitting all required documentation accurately and timely.

Job Requirements

Qualifications

• Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23-6-4-3.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

Pay Range

$26.70 - $39.71/hour
