Job summary

CommonSpirit Health at Home is seeking a Medical Social Worker for PRN assignments in Monument, Colorado Springs, Fountain and Manitou Springs. You will develop care plans with the physician, provide crisis intervention, and support patients and families where social factors influence medical outcomes.

The role requires a Master’s in Social Work, state licensure, at least one year of healthcare social work, and CPR/BLS.

Qualifications

• Master’s degree from an accredited school approved by the Council on Social Work Education.
• Currently licensed to practice in the state served.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross.

Responsibilities

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting in understanding social, emotional and economic factors influencing medical improvement.
• Providing patient care education to patients and caregivers.
• Assisting in making appropriate community referrals for public assistance, housing, health care resources, transportation.
• Working with caregivers to resolve problems interfering with care.
• Completing and submitting all required documentation accurately and timely.

• Establishing a care plan in accordance with the physician’s plan of treatment.

• Providing crisis intervention as required.

• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.

• Providing patient care education to patients and caregivers.

• Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)

• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.

• Completing and submitting all required documentation in an accurate and timely manner.

Pay Range

$28.10 - $46.37 /hour
