The University of Illinois Chicago (UIC) invites applications for the DSCC Core/Connect Care Associate in Rockford, IL. This full-time role coordinates care for families, conducts assessments, and supports eligibility and resource connections under supervision.

Responsibilities include developing person-centered care plans, coordinating visits, documenting activities within required timelines, and collaborating with Medicaid and DSCC teams while applying ADA accommodations as needed.

Qualifications

• Bachelor's degree with a major in the social or behavioral science or related health specialty from an accredited college or university.
• OR
• Master's degree in social or behavioral science or related health specialty area from an accredited college or university.
• One (1) year (12 months) of progressively more responsible full-time experience in the practice of social services/ agencies or medical/clinical setting maintaining acceptable standards of social work practices

Responsabilités

• Under the direction of the regional manager, performs active care coordination services by completing comprehensive health assessments, identifying families' strengths, and developing a person-centered care plan.
• Join and participate in Medicaid managed care clinical rounds, or other interdisciplinary care team meetings with participants’ providers, family members, or school teams.
• Educate, support, and connect families with resources for a seamless age transition and coordinate medical services, treatments, supplies, and environmental modifications.
• Assist families with private/public health insurance through effective benefits management practices for recipients.
• Contribute to quality improvement initiatives and participate in special projects as assigned.
