The purpose of an Oak Street Health Medical Scribe is to serve as a personal data assistant to the primary care provider. This role involves handling clinical data responsibilities during patient visits, including:

1. Observing and recording patient encounters/examinations
2. Documenting patient information, history, and diagnoses
3. Assisting in medical management
4. Documenting medical decision-making
5. Consulting with the care team and other providers on patient needs
6. Performing other duties as assigned
