Job description

Position Summary :

This position acts as a personal assistant to the physician, performing documentation in the EMR, gathering information for patient visits, and partnering with the physician to deliver high-quality patient care. The primary goal is to increase the efficiency and productivity of the physician, enabling them to focus on patient care.

Essential Functions :
  1. Prepare and assemble medical record documentation/charts for physicians.
  2. Enter patient room with the physician during visits to capture and transcribe documentation using electronic medical record applications.
  3. Ensure medical record compliance through self-documentation attestation.
  4. Update patient history, including history of present illness, review of systems, past medical and surgical history, and other relevant health information.
  5. Document physical examination findings as performed by the physician.
  6. Record times of patient care activities, including communications and re-examinations.
  7. Prepare and send documentation to physicians for review and approval.
  8. Monitor lab results and screening procedures.
  9. Comply with hospital and medical facility policies, including HIPAA and Joint Commission standards.
  10. Perform clerical duties to improve provider productivity and workflow.
  11. Keep detailed documentation on each physician's patients as per directives from VP of Operations and physicians.
  12. Attend scheduled work hours, including nights and weekends, following call-off procedures.
Work Settings :
  • Home-based, Assisted Living Facilities, Skilled Nursing Facilities/Rehab, Telemedicine, etc.
Physical Requirements :

Standing, walking, using hands, reaching, climbing, balancing, stooping, kneeling, talking, hearing, tasting, smelling, and lifting various weights up to 100 pounds, depending on the task.
