Clinical & Support Options, Inc. (CSO) seeks a Medical Respite Shelter Counselor for our Northampton program, a 10-bed facility helping guests heal and transition to housing.

You will coordinate assessments, treatment planning, and follow-up while advocating for benefits and coordinating services within CSO’s network. The role demands a background in social services (CNA/Med Assistant or related human services experience), a valid driver's license, and bilingual abilities are strongly

Qualifications

• High School diploma or GED is required.
• Two years' experience as a CNA or Medical Assistant OR two years' experience in the Human Services field.
• Valid driver’s license and reliable transportation.

Responsabilités

• Complete new guest assessments to determine eligibility and needs for social support programs.
• Assist guests in developing plans toward self-sufficiency and housing stability.
• Support ADLs and IADLs, including mobility, medications, grooming, meal prep, and appointments.
• Provide resources for budgeting, job training, and locating permanent housing.
• Advocate for guests with service providers and refer to agencies as needed.
• May provide transportation to appointments, interviews, and related meetings.
