Responsibilities

• Coordinate and review all medical records for the assigned caseload.
• Actively participate in Case Management and Treatment Team meetings.
• Serve as an educator to all departments.
• Coordinate discharge referrals as requested by clinical staff, fiscal intermediary, patients, and families.
