Job summary

Atrium Centers Gladwin Nursing & Rehabilitation Community is seeking a Transportation CNA to transport residents to medical appointments and other destinations safely. You will assist with transfers, mobility, and safety during transport, and perform pre-trip inspections, while maintaining accurate transportation records and following HIPAA and safety guidelines.

Qualifications

• Active CNA certification in the state where you work (required).
• Current CPR and First Aid Certification.
• Valid driver’s license in the state you work.
• Current 10-year DMV driving record.

Responsibilities

• Safely transport residents to hospitals and medical appointments.
• Assist residents with transfers, mobility, positioning, and safety during transportation.
• Perform pre-trip vehicle inspections and maintain safe, clean vehicles.
• Communicate with dispatch and supervisors about schedules and resident needs.
• Maintain transportation records and shift documentation.
• Follow safety, HIPAA, and confidentiality requirements.
• Report changes in schedules or vehicle concerns; respond to emergencies.
• Support resident care during transportation and other assigned duties.

What You’ll Do

Transportation CNA

• Safely transport residents to and from hospitals, medical appointments, dialysis, rehabilitation centers, and other destinations

• Assist residents with transfers, mobility, positioning, and safety during transportation

• Perform pre-trip vehicle inspections to ensure vehicles are safe, clean, and properly equipped

• Communicate with dispatch, supervisors, and team members regarding transportation schedules and resident needs

• Respond promptly and appropriately to resident needs, emergencies, and unsafe conditions

• Maintain accurate transportation records and shift documentation

• Follow all safety, sanitation, HIPAA, and confidentiality requirements

• Maintain a professional and caring demeanor when interacting with residents, families, visitors, and team members

• Report any changes in transportation schedules, resident needs, or vehicle concerns

• Perform other duties related to transportation and resident care as assigned

Medical Records

• Maintain accurate and up-to-date resident medical records

• Respond to medical records requests in a timely and professional manner

• Follow all documentation and record keeping policies, including vitals

• Report any violations of the Resident’s Bill of Rights immediately

What You Bring

• Active CNA certification in the state where you work

• Current CPR and First Aid Certification

• Valid driver’s license in the state where you work

• Current 10-year DMV driving record

• Ability to safely operate center vehicles and navigate transportation routes

• Ability to assist residents with mobility and transfers, including lifting, pushing, and pulling up to 50 pounds

• Strong communication and interpersonal skills

• Ability to remain calm and make sound decisions during emergencies

• Ability to follow oral and written directions

• Sincere and caring interest in serving older adults

• Ability to work cooperatively with residents, families, and team members

• Excellent attendance and reliability

• Must meet all required health, background, drug screening, and regulatory requirements
